Community High School District 94 is part of the Educational Benefit Cooperative. Through the Cooperative, District 94 employees have access to the following insurance options.
Health Insurance
Blue Cross and Blue Shield of Illinois provide three health insurance plan options for District 94 employees. PPO1 is a preferred provider option plan with no annual deductible. PPO2 is a preferred provider option with an annual deductible of $250 per person (in-network) and $500 per family (in-network). HMO is a health maintenance organization option. Prescription Coverage
For employees who have elected PPO1 or PPO2 health insurance, prescription drug coverage is provided by Medco. Further details are available in the benefit summary. Life Insurance
District 94 employees may elect supplemental life insurance through Reliance Standard. Please see Cheryl Glunt in the Human Resources office (x6215 or cglunt@d94.org) for initial enrollment forms. Vision Discount Program
Employees who elect on of the health insurance programs are eligible for a vision discount program through Davis Vision.
